Please register early – hotel and conference space are limited. Registration fee includes
welcome reception, Continental breakfasts with course faculty, refreshment breaks,
course materials, and certificate of attendance.

Guest Fee: $150 per family (Includes immediate family only—spouse and children under 18.)
Family Registration
Name badges are required for admission to breakfasts, Après Ski Refreshments, and the Saturday evening welcome reception. Limited to spouse or significant other and children under 18. Please PRINT all family names including name of registrant as you would like them shown on name badges. (Include ages of all children.)

Registration Fee Refund Policy
Refunds must be requested in writing by January 15, 2010, and will be subject to a $75 administrative fee. No refunds will be made on cancellations received after that date. Program materials cannot be guaranteed unless enrollment is received by January 15, 2010. Stanford University School of Medicine reserves the right to cancel this program; in the event of cancellation, course fees will be fully refunded.
